This is my field. This is what I deal with every single day, getting medical insurance claims paid and running up against insurances head to head when I need to. I do not know what state you’re in, but in NC, a patient can report questionable behavior and blatant wrongdoing from a provider directly to the State Medical Board as a complaint (http://www.ncmedboard.org/resources-information/consumer-resources/complaint-process), bypass all that nonsense with the insurance carrier.
Research the medical board information for your state, look into their provider complaint process. Also, look for your State’s Department of Insurance website. They should have a Consumer Assistance/Complaint process as well.
Most insurance carriers are only concerned about collecting your monthly premiums timely, managing your care or overseeing any abuse is not their focus. *shrugs* I hate to say that, but it’s the truth.
